All students planning to attend must complete and sign a Registration Form and Prom Promise. Guests listed on a student registration form must sign a Guest Prom Promise. All forms must be mailed together with payment. The Registration Form has a section which must be signed by a parent if the student will be bringing a guest as a "date". We realize all guests may not be dates; however, if that is the intention, the couple (a young man and a young lady) must have discussed with a parent the Behavior Expectations related to public displays of affection. The Prom Promises must be signed by both a parent and the student. Families with more than one student attending may use only one form, but all attending students in the family must sign the form.
